(1) according to MS Word this statement is wrong
These examples indicate my initiation, enthusiasm, communication, mentoring, motivating and leading skills.

(2) MS word suggested

These examples indicate my initiation, enthusiasm, communication, and mentoring, motivating and leading skills.

Here "and" came twice. is it not like 'and' has to come once?

(3) I think according to the GMAT we cant combine two different tenses like this. It can be like

These examples indicate my initiation, enthusiasm, communication, and mentor, motivate and lead skills.

Please clarify it.

Don't use MS Word for grammar lessons - it's a computer program, not an English teacher, and often gets things wrong. Neither sentence is a good one.

I'm not quite sure what you're trying to say - are you trying to give a list of attributes and skills? Try something like:

These examples demonstrate my initiative, motivation, and enthusiasm, as well as my leadership, communication, and mentoring skills.

(Although I would use just motivation and drop enthusiasm - enthusiasm is already a subset of motivation here.)
