A is out because the -ing modifier makes it appear as if John Wilmot Rochester is insisting himself ,however as per the meaning its charles II who is insisting !!

B is out for the same reason
C is out because parallism with "and " makes it faulty .in two parallel clauses the pronoun of the 2nd parallel clause refers to the subject of the previous clause and here its it gives a nonsensical meaning
D is right : insist that XY is a perfect usage (check manhattan sc edition 4 page 114 )
E is faulty because the usage is :insist that XY
plz share the OA and the source !!

1. We need a source in order to discuss problems (can't re-produce paid resources).

2. If this is a question from GMAT Prep, which we can discuss (free resource), you can re-post with a screenshot of the problem from Prep software on the proper forum.

3. You can ask questions about OG problems without posting the problem. Yes, you do nee a "that" in this idiom because if "x insists y", y is a thing. Instead, "x insists THAT y do something" is correct because you need a word (THAT) to allow us to reset the sentence and allow x to insists a process (clause w/ subj and verb).
