SC help

I am little bit confused about the comma separated sentence ,what does the comma separator sentence modifies ,whether it modifies the nearest noun of the preceding clause or the subject of the preceding clause.
for example:
Once the two parties reached an agreement,they began to work out the details of the settlement.
why the above one is incorrect.
and why the correct answer is given below.

Once an agreement was reached by the two parties ,they began to work out the details of the settlement.

Please given some more examples on this.

hi - what's the source of this problem? is this from our CAT exams?
